Prayer Request Form

The Catholic Community of St. James the Apostle prays for those who are in a time of need or have passed away. Complete the information below to include the name of the person whom you wish to be added to our prayer list and their name will be added to this month's list that will be placed on the altar to be prayed for during mass.

All of the requested information must be provided or the submission will be rejected as incomplete. Additionally, because the email address is verified to be authentic, an inaccurate email address entry will reject the submission.
